  • You may fill out this form online beginning September 14 at 9:30 a.m.  
  • The Library will not be responsible for lost, delayed, or undelivered email.
  • Children are registered according to date and time forms are received. Lists of the children in each group will be posted in the Children's Room on Saturday, September 26, 2009.
  • Please use a separate form for each child you are registering.
If you are registering a child for the first time, you must bring the following items to the Children's Room before Saturday, September 26, 2009 or risk losing your space:
  1. Copy of any document showing child’s name and date of birth and
  2. Copy of parent’s or child’s Garden City Library card.
